For your convenience, we also provide an e-mail address for your billing questions. Please carefully review the following agreements PRIOR to using this e-mail address, which can be found at the end of the agreements.

By using this e-mail communication, you are agreeing to the following:

- You understand that premier allergy “the Practice”, engages a third party company, “the Billing Company” for its billing.
- You hereby authorize the Billing Company or the Practice to send billing documents containing protected health information (PHI) of yourself or your dependent to your e-mail address.
- You attest that you are the patient or the patient’s authorized representative with authority to request and receive documents containing PHI.
- The Practice has informed you that its e-mail system is not encrypted. Consequently, there is an inherent risk of a third-party intercepting and viewing these e-mails and you accept this risk.
- You acknowledge this authorization is for a one-time use only.
- You agree and acknowledge this e-mail is not intended for use in seeking or rendering medical care and you agree not contact the Practice using this e-mail in the future as the practice does not routinely check this email address throughout the day. If you require medical care, please contact our office using your secured patient portal or call 303.468.8668 with any questions or concerns.
- You understand and acknowledge that the Practice may, in its sole discretion, refuse to send your PHI via e-mail despite your authorization. In such an event, an alternative method of communication will be offered.
- You acknowledge that e-mails communication become a part of your electronic health records.
