When you can’t make it to your appointment as planned

We understand that you may have an unexpected change in your schedule.   Please contact us as soon as possible to cancel or reschedule your appointment(s).   

Should an urgent situation arise within 2 business days before your scheduled appointment, simply give us a call.  We will find a different time on the same day of your appointment that better fits your new schedule.  

If you are under the weather or unable to travel to our office as scheduled, please call us, and we will change your appointment to a telehealth or a telephone visit so that you can keep your appointment from the comfort of your home.  Please review information on telehealth and telephone visits for more details.  

As we all know, traffic in our Denver metro area has increased as our community has grown.  If you are running late, let us know as your safety is our priority.  We will work to fit you in to our schedule on the same day.

To help us continue with providing comprehensive care for you and other patients, an appointment that is cancelled, rescheduled, or results in a “no-show” without 2 business days advanced notice and that is not converted into a telephone visit or telehealth visit will result in a $50 fee for an existing patient appointment and a $150 fee for a new patient appointment.  This fee will apply regardless of whether we receive your confirmation of your financial responsibility estimate and/or portal information.

Repeated cancellation and/or no-show of your appointments may result in dismissal from the practice.

We’d love to take care of your family

In order to allow adequate time to communicate with patients, we can only provide care to the patient who is scheduled for that appointment.  Even seemingly minor problems deserve our full attention during a scheduled appointment.   If you have other family members in need of medical attention, please inform our staff so that we can schedule additional appointments for them accordingly.

Prescription Refills

Most of your prescriptions from premier allergy can be refilled for up to 12 months from your last appointment.  Please contact your pharmacy for additional refills first.  

If a refill prescription is required, send us a request using the secure patient portal, or call us.  The following information helps us expedite your request:

  • Patient’s full name 
  • Patient’s date of birth 
  • Pharmacy information (Name, phone number, and location of the pharmacy) 
  • Please clearly state the medication names, dose and frequency.
  • For children, please notify us of certain formulations that the patient may prefer, such as chewable tablet or syrup.  If no such information is provided, we will use our best judgment in choosing the formulation for your child.

Please monitor your medication supply, and allow us adequate time to address your medication needs.  No prescription requests will be refilled outside our normal business hours unless it is an emergency situation.  We will notify you if a follow up visit is required prior to refilling medications.  Regrettably, we cannot complete refill requests for patients who have not been seen in the past 12 months.

Forms 

Our providers carefully review the chart to individualize forms for each patient.  This ensures accuracy and appropriate continuity of care for our patients, whether it is at your child’s school or your work place.  An office visit is usually required to complete these forms with the most updated information.   

Please provide us with the forms you need.  We do not provide any access to internet, printing, or copying for these forms at our office.

For school forms or camp forms, help us by planning ahead for your upcoming visit.  Bring the forms in with you during your annual evaluation or follow up visit, and our providers will complete them as part of the visit at no extra charge.  

If your child’s last appointment with us was within the past 6 months and there is no change in his or her health, simply send in the form.   Our providers will review your chart and complete the form for a $5 fee per request.  

An office visit is required to complete FMLA forms.  Fees for other forms or letters will be assessed individually based on their requirements.

Regrettably, we cannot complete forms for patients who have not been seen in the past 12 months.  An office visit will be required in order for us to safely and accurately complete the form(s).

Test/Treatment

Diagnostic testing (“tests”) may be recommended to find the root cause(s) of your symptoms.  Our providers’ decision on which tests are necessary and appropriate as well as safe for you depends upon many different factors and is generally only determined after a comprehensive clinical assessment.  To ensure optimal results and safety, please schedule your tests as soon as possible.  A follow up visit may be required if the recommended testing is not performed in a timely fashion.   The time frames below offer general guidance as to the maximum amount of time that would be considered reasonable between clinical evaluation (a provider visit) and testing.

Immunotherapy is a very effective treatment for environmental allergies.  It carries a small but real risk of life-threatening reactions.  Our providers will discuss treatment plans that will lower this risk during your office visit.  Certain medical conditions will also increase your risk for life-threatening reactions, and our provider will assess these conditions to ensure that they are stable immediately prior to starting immunotherapy.  A follow up visit will be required if immunotherapy is not started within 2 months of your last office visit.

Virtual Visits

Our practice embraces advanced technologies as we believe that they allow us to provide better care and a better experience for our patients.  We currently offer 3 types of virtual visits at premier allergy

  • Audio-visual telecommunications often referred to as “Telehealth”
  • Audio-only telecommunications often referred to as “Telephone visit”
  • Online communications through a secured patient portal often referred to as “e-visits or electronic visits”

IMPORTANT POINTS TO REMEMBER:

  • Except for telephone visits, for your safety, please do not use telehealth or e-visits if your situation is life-threatening or requires urgent attention. 
  • Prior to your appointment, please send us an update on any changes in your health status.  
  • This includes any new diagnosis, surgery, allergies, hospitalization, social, or family history.  You may also directly update this information in your patient portal.
  • View your current medication list on your secure patient portal.  If you are taking any new medications or need to update your current medications, please send us this information.  
  • Unless we receive your updated health information, our providers will make a treatment plan for you based on an assumption that your health information has remained the same since your last visit with us. 
  • These virtual visits are integrated in and become a part of your electronic health record.

As convenient as these technologies are, we recognize that nothing replaces an in-person face to face visit between a patient and a provider.  Our providers may need to evaluate your request for these virtual visits before you can make this type of appointment to ensure that we can safely care for you using this technology.   

We do not recommend telehealth or e-visits if you are experiencing symptoms that can be life-threatening or if you require testing that needs to be performed in our office in order to complete the visit.  

For patients who have established care with us, you must have had at least one in-person visit with one of our providers in the past 12 months to be eligible for these virtual visits.

Please be sure that your device or computer functions prior to your visit.  A telehealth or telephone visit that is cancelled or rescheduled due to a failure of your device or connection will be subject to the same rescheduling, cancellation, and no-show policies as in-person appointments.

It is important to remember that after your virtual visits, our provider may still advise you to return to our office for a regular in-person visit.
